Provider Screening & Verification Notice

StopLeak911 may verify the information provided in this application, including business identity, Responsible Master Plumber licensing, insurance status, service-area eligibility, public disciplinary or complaint information, and other lawfully available public records. Verification may be repeated while a provider remains in the network.

Screening and verification are risk-reduction measures only. They are not a guarantee of a provider’s character, future conduct, workmanship, safety, licensing status at the time of a particular job, or fitness for every assignment. Providers remain independent businesses responsible for their own employees, subcontractors, licensing, insurance, permits, work, pricing, and legal compliance.

If StopLeak911 later uses a third-party consumer-reporting agency to obtain a criminal or other consumer report, any separate disclosure, authorization, or other process required by applicable law will be provided before that report is ordered.

Submitting an application does not guarantee dispatch activation. Provider approval may be withheld, suspended, or revoked when credentials cannot be verified or no longer meet network requirements. This screening notice is not a substitute for any separate consumer-report disclosure or authorization required by law.
